I, the undersigned, want to enroll (or enroll my child) in the Snö Mountain Terrain Park Slopestyle Competition and realize that freestyle snowboarding and skiing is an action sport carrying significant risk of personal injury.  I understand that participation in the program is totally voluntary on my part (or on the part of my child).  I know that there are natural and man-made obstacles or hazards, surface and environmental conditions, and risks which in combination with my actions can cause me severe or occasionally fatal injuries.  I agree that I, as a participant (or as a parent/guardian of a participant), must take an active roll in understanding and accepting these risks, conditions, and hazards.  I further agree to release, hold harmless, and grant complete indemnification to Snö Mountain Ski School, The Ski Corner, Snö Mountain Ski Area, its employees, instructors, directors, agents, sponsors, volunteer workers, and assignees should any of the above occur.

 I, the undersigned, give unrestricted right and permission to copyright and use, re-use, publish, and republish video of me made through any and all media now or hereafter known for illustration, art promotion, advertising, trade or any other purpose whatsoever.
